The Hypertension Remote Patient Monitoring program at the Maternal Fetal Medicine Clinic at UWMC Montlake campus is run by the Maternal Fetal Medicine (MFM) medical doctors and Advanced Practice Registered Nurses (ARNP) team and is supported by high-risk clinic Registered Nurses.

What is remote patient monitoring?

Remote patient monitoring (RPM) is a type of telehealth service where healthcare providers monitor patients outside traditional care settings.

With RPM, patients' blood pressures can be evaluated and managed from their own home. They use a monitor to measure their blood pressure and then electronically record this into the patient portal through MyChart. Blood pressure readings are regularly reviewed by an expert team that makes recommendations regarding medication adjustments. Patients also consult with one of our specialists, typically over telemedicine, to learn more about blood pressure changes postpartum, and how blood pressure complications in pregnancy may impact subsequent pregnancies and their future health.  After completion of postpartum monitoring through the RPM program, we encourage patients to seek regular follow-up with a primary care provider.

 

Why is monitoring blood pressure after delivery important?

Individuals with high blood pressure during pregnancy are at risk for worsening hypertension in the first few days to weeks after delivery. Blood pressure changes in the postpartum period can be unpredictable. Adding, changing, or stopping blood pressure medications  is common during postpartum, and has traditionally required frequent visits to the hospital or clinic to see a healthcare provider. With RPM services, we are able to do much of this remotely.

blue wristband graphicAs part of the RPM program, patients with or at risk  for preeclampsia after delivery may receive a blue wristband at hospital discharge. This band is a simple way to remind you, your care team, and emergency medical care to monitor your blood pressure closely during the postpartum period.

 Costs associated with Remote Patient Monitoring

  • Patients may need to purchase a blood pressure cuff to participate in this program. We believe it’s highly valuable for them to have their own blood pressure cuff for their long-term health.
  • MyChart access is free for all University of Washington patients.
  • Remote Patient Monitoring (RPM) services, like all other care services, are billed to patients’ insurance. These services will be noted by any of the CPT codes: 99453, 99454, 99457, 99458, or 99091. Insurance: In general, RPM services are covered by insurance. Patients should always confirm with their insurance plan if they have questions about coverage.
