Please fill out and email all these items to Kari Schubarth 3-weeks prior to the start.
- Jpeg photo for ID/badge.
- Full name with middle initial
- Your immunization records including TB status and flu vaccine (during relevant months).
Privacy / security agreement- Network sign on form for non-Benefis entities

Apartments within walking distance to the Hospital are provided. They are furnished with full or queen sized beds. Basic kitchen supplies, cleaning supplies, sheets, towels, vacuum cleaners, irons and ironing boards are provided. There are coin-operated washers and dryers in the apartment complex. Please note: although you will have a private room, you may be sharing the apartment with other WWAMI students on rotation in Great Falls. Please email Kari Schubarth for apartment details.
First Day
Plan on meeting Kari at 8:00 am in the South Tower, Entrance #2 of Benefis Hospital–East Campus. A map of Benefis Hospital is available online at www.benefis.org – (Click on Patients and Visitors, then East and West Campus Maps). She will give you your badge and take you to your EMR (Electronic Medical Records) training in the NextGen Room at Benefis Hospital. Training will take approximately one hour and then you will report to Dr. Palen.
